Medical / Conference Visa
Visa requirments
General Requirements:
For Conference Visa:
a) Address in Pakistan should match with that of the NADRA card
b) Telephone no.'s of applicant and of the host in India are mandatory.
c) Validity of the passport should be at least 6 months from the date of deposit of the application.
d) All previous passports should be attached (not necessary if the applicant has traveled on the present passport to India.)
e) Pakistani of dual nationality MUST apply on their PAKISTANI passports.
f) Four filled forms in typed format (only Capital Letters)
Supporting Documents For Conference Visa
1. Copy of the NADRA card along with English translation clearly indicating the address.
2. Letter of invitation from the organizers in India.
3. Letter of introduction from the organization the applicant is working in.

For Medical Visa:
a) Address in Pakistan should match with that of the NADRA card.
b) Telephone no.'s of applicant and of the host in India are mandatory.
c) Validity of the passport should be at least 6 months from the date of deposit of the application.
d) All previous passports should be attached (not necessary if the applicant has traveled on the present passport to India.)
e)It may be noted that a maximum of two attendants are allowed along with patient.
f) Four filled forms in typed format (only Capital Letters)
Supporting Docs for Medical Visa:
1. Copy of the NADRA card along with English translation clearly indicating the address.
2. Letter of appointment with a recognized hospital in India.
3. Medical records of patient in Pakistan clearly indicating diagnosis.
Note:
If any column in the application form is left blank or not filled in complete, the application would be rejected.
